A pre-populated form keeps appearing whenever I choose "Create" from the sidebar of my Form Workflow spreadsheet. I cannot seem to create a blank form from a blank spreadsheet. What is the solution?

1. Create your form (Go to Tools, then choose Create a form)
 
 
2. Start the add-on Form Workflow
 
Your form will already be linked so you can go straight to Form Workflow activation.
 
 
 
 
3. Turn Workflow activation ON from the Setup tab
