A pre-populated form keeps appearing whenever I choose "Create" from the sidebar of my Form Workflow spreadsheet. I cannot seem to create a blank form from a blank spreadsheet. Start the add-on Form Workflow Your form will already be linked so you can go straight to Form Workflow activation. 3.
