Click Create. A new tab will appear on your spreadsheet named Form Responses
Click on the menu "Form" then choose Edit form.
From this form, you will be able to set up your questions and your preferences.
You can also change the theme, view the responses, and see the live form.
Choose Anyone in [Domain] if this form is only to be used within your GSuite domain (1).
Check the first box if you want to collect respondent's username (2).
Turn Workflow Activation On when you have finished editing your Google Form.
